C. L. Brumback Primary Care Clinics’ Staff Complete Community Health Worker Training

Four Patient Navigators from the C. L. Brumback Primary Care Clinics completed their Community Health Worker (CHW) training with the Palm Beach County Medical Society On May 19, 2018. The program trains local patient navigators to improve chronic disease management programs, health insurance enrollment, immunization drives, HIV/AIDS treatment, access to mental health services and maternal-child health interventions within their organizations. The four navigators from the Delray Beach, Jupiter, Lantana, and West Palm Beach Clinics are the second group of program graduates in two years. With this certification, they are now able to sit for the Certification for Community Health Workers through the Florida Certification Board. The Patient Navigators serve adult and pediatric patients who receive medical, dental and pharmacy services at the clinics, Federally Qualified Health Centers that are owned and operated by the Health Care District of Palm Beach County.
