Advisory Board or Committee

The Health Care District would like to thank you for your interest in volunteering your time to serve on one of its advisory boards or committees. The Health Care District and its subsidiaries strive to ensure that all organizational decisions are made solely to promote the best interests of the District, its patients, members and residents without favor or preference based upon prohibited personal considerations. Therefore, we ask that you please fill out and sign the Application for Appointment, Conflict of Interest Form, and Ethics Pledge and email or mail those, with a copy of your curriculum vitae (CV) or resume, to the addresses listed below.
