District Cares Specialty Benefits
The Health Care District of Palm Beach County administers the District Cares specialty benefits for eligible patients of the C. L. Brumback Primary Care and Dental Clinics. The specialty benefits are available to uninsured residents of Palm Beach County who do not qualify for any other public assistance health coverage program and who meet income and residency requirements. You will meet with a financial counselor who will assist you in applying for Medicaid and/or marketplace programs/products. Once the denials are received from those programs, then District Cares Specialty Benefits will be an option.
All specialty care visits must be ordered by a C.L. Brumback Primary Care Provider. Specialty care must be provided by a participating physician. The District Cares Specialty Voucher Program includes:
- Inpatient – Acute Care
- Inpatient – Rehabilitation
- Outpatient – Cardiac Rehabilitation
- Emergency Room Services
- Outpatient – Diagnostics
- Outpatient – Surgery
- Outpatient – Therapy
- Durable Medical Equipment / Supplies
- Home Care / Home Infusion
- Laboratory Services
- Specialty Provider Services (i.e. Cardiology, dermatology, hematology, oncology, endocrinology, pulmonology, perinatology, gastroenterology, rheumatology, nephrology, infectious disease, ophthalmology, otolaryngology, podiatry, gynecology, orthopedics, otolaryngology/ENT, and urology)
A patient should first call to schedule an appointment to see a primary care provider in the C. L. Brumback Primary Care Clinics (at 561-642-1000). During the call to schedule an appointment, the Clinic registration team will begin the financial assessment process by advising the patient to bring proof of Palm Beach County residency, personal identification, and income information*.
*Qualifying applicants must have an income equal to or less than 100 percentage of the Federal Poverty Guidelines(PDF).
Once a patient schedules an appointment with the C.L. Brumback Primary Care Clinic they will establish their medical home. Their primary care physician will coordinate comprehensive patient treatment with a team of care providers based on a patient’s individual medical needs. Care will move seamlessly as needed between primary care physicians, medical specialists, hospitals and other services.
