We accept patients who walk in. We recommend, however, that patients call 561-642-1000 to make an appointment.

Prior to the appointment, patients can download, print and complete the registration form and bring it with them to the appointment. Patients should also bring proof of their address, a copy of their health coverage card if they have it and any medical records from other doctors or hospitals. They can also bring a list of the medications they are currently taking, a list of their health concerns and three most recent pay stubs information to apply for our Sliding Fee Scale if uninsured.
