The Health Care District of Palm Beach County’s custom-designed Mobile Health Clinic fleet is the newest edition to the C. L. Brumback Primary Care Clinics. Our two 38-foot buses, Warrior and Hero, as well as our third, smaller mobile unit, Scout, are hitting the road to deliver medical and behavioral health services to the chronically homeless and other underserved adult patients across the county.
Our Mobile Health Clinics provide medical and behavioral health service including general medicine, psychosocial assessments by a licensed clinical social worker (LCSW) or a licensed mental health counselor (LMHC) by telehealth, telepsychiatry and pharmacy services. By offering a “one-stop-shop” model of comprehensive medical care at sites throughout the county, our clinics on wheels are expanding much-needed access to health care for the homeless by meeting them where they are. The Mobile Health Clinics partner with community agencies that traditionally provide other social services at these locations, such as food, showers, housing, case management, etc. The ultimate goal is to establish the C. L. Brumback Primary Care Clinics’ brick-and-mortar locations as the medical home for these patients on their road to recovery and a sustained healthy lifestyle.

- Primary care services (physical exams, chronic disease management (diabetes, hypertension, etc.), preventative care (colorectal and cervical cancer screenings, mammogram referrals, etc.), behavioral health services (screenings, psychiatric services, addiction treatment, etc.)

- Serving all patients with or without insurance and regardless of ability to pay (a sliding fee scale is available based on family size and income)
- Two fully-equipped exam rooms (hydraulic exam bed for bariatric patients), counseling room, ADA compliant with wheelchair-accessible bathroom and hydraulic wheelchair lift, three TV monitors for patient education, large exterior electric retractable awning to provide shade and wireless access to the Health Care District’s Electronic Health Record System

Community Partnership Schools
As part of the Community Partnership Schools program, the Health Care District's mobile "clinic on wheels" Warrior is parked at two Palm Beach County schools to provide access to comprehensive medical services including well and sick
visits, sports physicals, medication refills, immunizations and more.
