There are legal exceptions to the rule. The hospital will only release private information when permitted or required by law.

The patient must sign a permission form before PHI can be used or shared. PHI is used for treatment, payment, is health care related or when required by law.
The permission form must be fully completed. It must be signed and dated. It can only be signed by the patient or the patient's personal representative. PHI cannot be used or shared without the signed form.
