District Cares Specialty Benefit
The District Cares Specialty Benefits extends the care provided by the CL Brumback Primary Care Clinics to eligible residents of Palm Beach County with access to specialty care providers including physician specialists, diagnostics, laboratory services, and hospital services..
Who is Eligible?
The program is available to CL Brumback Primary Care Clinic patients who are uninsured and not eligible for any other public assistance health coverage program and who meet income and residency requirements.
How to Apply
The patient should first call to schedule an appointment to see a primary care provider at the CL Brumback Primary Care Clinics. During the call to schedule an appointment, the Clinic registration team will begin the benefit qualification process. The patient will be directed to bring documentation to their first visit that will include:
- Proof of Palm Beach County residency (Current Drivers License, Electric or Phone Bill, Lease or Rent Receipt, etc.)
- Personal Identification (Current Drivers License, Social Security Card, US Certificate if Naturalization, Passport, etc.)
- Income (Documentation that show the prior 4 weeks of money you make like a Wages/Paystub, Unemployment, AFDC, Child Support, Workers Compensation, etc.)
*Qualifying applicants must have an income equal to or less than the allowed percentage of the Federal Poverty Level Guidelines (PDF).
