The Health Care District provides health care coverage to Palm Beach County residents in need.

		Coordinated Care Program--A Managed Care Program For those who qualify (must be a Palm Beach County resident meeting basic income and asset requirements), members receive access to primary care, specialty care, emergency care, hospitalization and prescription drugs. There is no co-payment for services.
		Healthy Palm Beaches, Inc. Personal Health Plan--A Medicaid HMO Palm Beach County Medicaid recipients may choose to join Personal Health Plan, the Medicaid HMO offered by Healthy Palm Beaches, Inc.
		Vita Health is a new and affordable health coverage option for you and your family. This program is a shared cost health plan that enables you and your family to access health care services that help build healthy lifestyles!
