Please visit the Hours and Locations page to find out the hours for the clinic that you're looking to visit.

We accept patients who walk in. We recommend, however, that patients call 561-642-1000 to make an appointment.

Patients should bring proof of their address, a copy of their insurance (if they have it), any medical records from other doctors or hospitals and a list of their health concerns. They should also bring all medicines they are taking in their original containers, including medicines prescribed by doctors and any creams, vitamins and products taken for pain, allergy, cold, etc. If patients wish to apply for the Sliding Fee Scale, they should bring either their most current tax return or three most recent pay stubs.

Our medical staff can help coordinate your care regardless of your ability to pay or whether you have health coverage.

The clinics are staffed with board-certified physicians, along with Registered Nurses, Advanced Registered Nurse Practitioners and Certified Medical Assistants, who provide adult and pediatric medical care to all residents of Palm Beach County using an individual care plan.

Our medical staff provides adult services that include: yearly physicals, medical evaluations and disease management, lab testing and mammogram referrals, diabetes/hypertension screenings, pharmacy services, health care for the homeless and mental health services.

Our pediatric clinic provides: well and sick child visits, school and sport physicals, childhood immunizations, hearing and vision testing, lead screening and lab testing.

Each site has a designated Patient Financial Counselor and a Case Manager to refer patients to other medical and social services.

Multilingual interpreters are available at our clinics.

The C. L. Brumback Primary Care Clinics want to be your medical home for all of your primary care needs.

Florida Health Palm Beach County (formerly the Palm Beach County Health Department) continues to provide other services including maternity check-ups and prenatal and postnatal services, family planning, immunizations, dental services, nutrition, WIC services and Dental Services. It's common for patients to access services at both the clinics and at the health centers.

Our clinics have Certified Application Counselors available to help patients interested in obtaining health coverage enroll through the Affordable Care Act's new federal health exchanges.

Health coverage eligibility staff are also available on-site to assist patients who may qualify for assistance of the Health Care District's health coverage programs.

Please see the Accepted Medical Plans page.
